Mogelijk vaccin tegen malaria in ontwikkeling

In a daring experiment in Europe, scientists used mosquitoes as flying needles to deliver a "vaccine" of live malaria parasites via their bites.

The results were astounding: Everyone in the vaccine group acquired immunity to malaria; everyone in a non-vaccinated group did not, and developed malaria when exposed to the parasites later.

On 7 July 2009, the Strategic Advisory Group of Experts (SAGE) on Immunization held an extraordinary meeting in Geneva to discuss issues and make recommendations related to vaccine for the pandemic (H1N1) 2009.

SAGE reviewed the current pandemic situation, the current status of seasonal vaccine production and potential A(H1N1) vaccine production capacity, and considered potential options for vaccine use.

The experts identified three different objectives that countries could adopt as part of their pandemic vaccination strategy:

protect the integrity of the health-care system and the country's critical infrastructure;
reduce morbidity and mortality;

and reduce transmission of the pandemic virus within communities.

Countries could use a variety of vaccine deployment strategies to reach these objectives but any strategy should reflect the country’s epidemiological situation, resources and ability to access vaccine, to implement vaccination campaigns in the targeted groups, and to use other non-vaccine mitigation measures.

Although the severity of the pandemic is currently considered to be moderate with most patients experiencing uncomplicated, self-limited illness, some groups such as pregnant women and persons with asthma and other chronic conditions such as morbid obesity appear to be at increased risk for severe disease and death from infection.

Since the spread of the pandemic virus is considered unstoppable, vaccine will be needed in all countries. SAGE emphasized the importance of striving to achieve equity among countries to access vaccines developed in response to the pandemic (H1N1) 2009

***The following recommendations were provided to the WHO Director-General:

All countries should immunize their health-care workers as a first priority to protect the essential health infrastructure. As vaccines available initially will not be sufficient, a step-wise approach to vaccinate particular groups may be considered. SAGE suggested the following groups for consideration, noting that countries need to determine their order of priority based on country-specific conditions: pregnant women; those aged above 6 months with one of several chronic medical conditions; healthy young adults of 15 to 49 years of age; healthy children; healthy adults of 50 to 64 years of age; and healthy adults of 65 years of age and above.

Since new technologies are involved in the production of some pandemic vaccines, which have not yet been extensively evaluated for their safety in certain population groups, it is very important to implement post-marketing surveillance of the highest possible quality. In addition, rapid sharing of the results of immunogenicity and post-marketing safety and effectiveness studies among the international community will be essential for allowing countries to make necessary adjustments to their vaccination policies.

In view of the anticipated limited vaccine availability at global level and the potential need to protect against "drifted" strains of virus, SAGE recommended that promoting production and use of vaccines such as those that are formulated with oil-in-water adjuvants and live attenuated influenza vaccines was important.

As most of the production of the seasonal vaccine for the 2009-2010 influenza season in the northern hemisphere is almost complete and is therefore unlikely to affect production of pandemic vaccine, SAGE did not consider that there was a need to recommend a "switch" from seasonal to pandemic vaccine production.

WHO Director-General Dr Margaret Chan endorsed the above recommendations on 11 July 2009, recognizing that they were well adapted to the current pandemic situation. She also noted that the recommendations will need to be changed if and when new evidence become available.

SAGE was established by the WHO Director-General in 1999 as the principal advisory group to WHO for vaccines and immunization. It comprises 15 members who serve in their personal capacity and represent a broad range of disciplines from around the world in the fields such as epidemiology, public health, vaccinology, paediatrics, internal medicine, infectious diseases, immunology, drug regulation, programme management, immunization delivery, and health-care administration.

Additional participants in the SAGE meeting included members of the ad hoc policy advisory working group on influenza A(H1N1) vaccine, chairs of the regional technical advisory groups and external experts. Observers included industry representatives and regulators who did not take part in the recommendation process in order to avoid conflicts of interest.

DEATH TOLL FROM INFLUENZA OUTBREAK NEARS 800, UN HEALTH AGENCY SAYS (UN: New York: July 24, 2009)

Influenza A(H1N1) is continuing to spread through all corners of the world, the United Nations World Health Organization (WHO) reported today, putting the global death toll from the pandemic at around 800.

Some 160 countries and territories have now reported laboratory-confirmed cases of the virus, but how it could potentially change over the coming weeks is still unknown, WHO spokesperson Gregory Hartl told reporters in Geneva.

Although five isolated cases of anti-viral resistance have been reported, no changes to the virus’ behaviour have been detected for now, he added.

Mr. Hartl warned that A(H1N1) is expected to increase this winter in the Northern Hemisphere due to the colder weather, but he underscored that WHO cannot predict the death toll over the coming months.

The highest percentages of deaths from the pandemic are among adolescents and young adults, he said, mostly likely because it spreads more quickly in schools and institutions.

On vaccines against A(H1N1), the spokesperson said that each manufacturer is working at their own pace, with clinical trials only having been started recently.

But WHO still expects the first doses to be ready by early fall in the Northern Hemisphere, he added.

Two manufacturers have guaranteed that 150 million doses will be made available, and the agency is pressing other partners to secure more doses, Mr. Hartl noted.

WHO plans to send the bulk of them to the least developed countries, with health-care workers, who work on the frontlines and are the most exposed to the virus, to be given priority.

Last month, the agency raised the alert level for A(H1N1) to Phase 6, the highest on its pandemic alert scale.

However, it stressed that Phase 6 refers to the spread of the virus and not its severity, and that the upgrade means that sustained human-to-human transmission of the virus has spread beyond North America, where it was initially concentrated.

Mogelijk Britse soldaat met Mexicaanse griep in Vierdaagse Nijmegen

Een Engelse militair in de Vierdaagse heeft mogelijk de Mexicaanse griep. Dat melden verschillende kranten.

Begin juli vroeg een viroloog zich af of het evenement, met ruim 40.000 deelnemers uit binnen- en buitenland, wel moest doorgaan. Volgens hem zou het ‘een geweldige bron zijn voor de introductie in Nederland van de Mexicaanse griep’.

In de loop van de dag wordt bekend of de soldaat werkelijk Mexicaanse griep heeft. Hij is nu geïsoleerd.

In de Volkskrant: Britse soldaat wandelde met Mexicaanse griep in Vierdaagse

EenVandaag meldt: “Als de man daadwerkelijk de gevaarlijke griepvariant heeft zal de organisatie extra maatregelen nemen, maar welke dat zijn blijft onbekend.”

Tot de uitslag heeft de militair een eigen kamer gekregen op Heumensoord. Medisch personeel van defensie verzorgt de Brit. Opname in het ziekenhuis was niet nodig, aldus een woordvoerster van defensie woensdagochtend.

De gemeente Nijmegen bevestigt die lezing. Woensdagmiddag of morgen wordt de uitslag van het onderzoek verwacht. In EénVandaag een gesprek met de organisatie en viroloog Ab Osterhaus.

In Heumensoord zijn vijfduizend militaire deelnemers aan de Vierdaagse gehuisvest, aldus EenVandaag. De soldaten reageren vooralsnog gelaten op het nieuws, aldus de defensiewoordvoerster.

Wereldwijd is het aantal doden als gevolg van de Mexicaanse griep gestegen tot boven 700. Dat maakte de Wereldgezondheidsorganisatie (WHO) dinsdag bekend.

De Argentijnse minister van Gezondheid Juan Manzur zei maandag dat het zogeheten H1N1-virus intussen 163 levens in het Zuid-Amerikaanse land heeft geëist.

Landen zouden kunnen overwegen scholen te sluiten om verdere verspreiding tegen te gaan. Dat heeft de WHO gezegd Vorige maand verklaarde de WHO de griep tot een pandemie.

In laboratoria zijn tot nu toe 125.000 gevallen van de ziekte bevestigd. In meer dan 125 landen is het virus opgedoken.

Twee weken geleden lag het dodental volgens de VN-organisatie nog op 429.

Canadees vaccin tegen HIV/AIDS gereed om op mensen te testen

Een vaccin tegen AIDS/HIV dat is ontwikkeld in Canada is door de veiligheidstesten op dieren gekomen. Dat bericht de Canadese nieuwszender CBC.

De ontwikkelaars ervan wachten nu op toestemming om het op mensen te testen in de VS. Een professor in de microbiologie, die 20 jaar aan het vaccin heeft gewerkt, zegt dat het een belangrijke mijlpaal is. Tot nu toe waren vaccins tegen AIDS/HIV niet succesvol bij mensen.

“Dozens of HIV vaccines have already been developed and tested in animal models but only a handful have been tested in humans, none successfully. (Canadian Press)”

Een eerder vaccin dat werd getest en dat door Merck and Co. werd ontwikkeld zorgde er juist voor dat meer mensen de aandoening opliepen dan de mensen die een placebo toegediend kregen.

CBC meldt: “A promising trial in 2007 by pharmaceutical giant Merck and Co. was shut down after those receiving the vaccine contracted HIV at a higher rate than those who received the placebo.”

Volgens een rapport van de Verenigde Naties over de wereldwijde AIDS-epidemie waren er 33 miljoen mensen met HIV in 2007. Twee miljoen mensen overleden dat jaar aan aandoeningen die in verband staan met deze ziekte.

Noodmaatregelen in Argentinië vanwege varkensgriep, wereldwijd 311 doden

In de Argentijnse hoofdstad Buenos Aires zijn noodmaatregelen getroffen om een uitbraak van de Mexicaanse griep te bedwingen. Dat meldt De Volkskrant.

De schoolvakantie is verlengd en de inwoners zijn verzocht grote openbare bijeenkomsten te vermijden.

In Argentinië zijn 1587 griepgevallen gemeld. Al 26 mensen vonden er de dood door het nieuwe griepvirus.

Herkomst virus?
Zieke varkens op een show in 1918 en een ongeluk waarbij wetenschappers in 1977 een “uitgestorven” virus vrij lieten komen hebben een hoofdrol gespeeld in de nieuwe variant van het varkensgriepvirus, zo schrijft de Pittsburg Tribune.

Dit hebben onderzoekers van de universiteit in Pittsburg gemeld. Mensen ouder dan 52 lijken enige immuniteit te hebben tegen het nieuwe virus, omdat ze voor 1957 resistentie hebben opgebouwd.

De Wereldgezondheidsorganisatie (WHO) van de Verenigde Naties heeft bekendgemaakt dat meer dan 100 landen te maken hebben met de Mexicaanse griep.

Er zijn ruim 70.000 mensen besmet en er zijn 311 mensen overleden. De VN geeft een tabel met al de landen en aantal besmettingen. In hoog tempo komen er nu besmettingen bij.

“Miljoen Amerikanen besmet met Mexicaanse griep”, kinderen ernstig getroffen

Volgens wiskundige modellen van het Amerikaanse Centrum voor Ziekten Controle zijn er momenteel al ongeveer 1 miljoen mensen in de VS besmet met het H1N1 influenza virus. Dat meldt de Los Angeles Times.

Er zijn al 3.065 ziekenhuis opnames en 127 doden in de VS. Een toenemend aantal mensen komt in het ziekenhuis terecht.

Het virus verspreidt zich nu ook snel over het zuidelijk halfrond.

Er is gemeld dat er minstens 1 varken besmet is. De jongste kinderen worden het ergst getroffen.

UN puts Southern hemisphere on alert for A(H1N1) virus

UN HEALTH AGENCY CHIEF PUTS SOUTHERN HEMISPHERE ON ALERT FOR A(H1N1) VIRUS

Health authorities around the world need to be vigilant as the influenza A(H1N1) virus meets seasonal flu viruses flowing through the yet unaffected Southern Hemisphere, warned the head of the United Nations World Health Organization (WHO) today.


“The current winter season gives influenza viruses an opportunity to inter-mingle and possibly exchange their genetic material in unpredictable ways,” WHO Director-General Margaret Chan said in her closing remarks to the World Health Assembly in Geneva.

One month after the outbreak of the new flu, WHO expects it to continue spreading to new countries and within nations already affected over the coming weeks.

According to WHO, as of this morning, some 42 countries had officially reported 11,168 cases of the infection, including 86 deaths.

However, Dr. Chan noted that the pandemic alert level remains at Phase 5 – on a six-point warning scale – meaning that sustained human-to-human transmission of the virus on a community level is mostly restricted to a single geographic region, in this case North America.

During the high-level consultation on pandemic influenza, several delegations called on WHO to consider criteria other than geographical spread when evaluating the phases of influenza pandemic alert.

Dr. Chan stressed that up to now, the new virus has largely circulated in the Northern Hemisphere, where epidemics of seasonal influenza should be winding down.

“We need to watch the behaviour of A(H1N1) very carefully as it encounters other influenza viruses circulating during the winter season in the Southern Hemisphere,” she said. “The current winter season gives influenza viruses an opportunity to inter-mingle and possibly exchange their genetic material in unpredictable ways.”

Although WHO does not predict a dramatic jump of severe or fatal infections, Dr. Chan warned that countries where the virus is already widespread should expect to see more cases of this type of infection.

“But countries, especially in the developing world, where populations are most vulnerable, should prepare to see more than the present small number of severe cases, which are being picked up under the best detection and testing conditions possible.”

The week-long Assembly gathered officials from WHO’s 193 member countries for an annual review of the agency’s activities and to set new priorities for the future. On its agenda this week were implementation of the International Health Regulations; primary health care, including health system strengthening; social determinants of health; and monitoring the achievement of the health-related Millennium Development Goals (MDGs).

De Volkskrant meldt intussen dat in Mexico-stad al een week geen nieuwe griepbesmettingen meer geconstateerd zijn.

“Het is volgens burgemeester Marcelo Ebrard niet meer nodig in het openbaar mondkapjes te dragen. Hij raadt de bevolking wel aan op drukke plaatsen als scholen en de metro hygiëne in acht te nemen en een 'gezondheidscultuur' uit te dragen,” zo meldt de krant.

Eerste dode in VS door varkensgriep, Mexicaanse baby overleden in Texas

In de Verenigde Staten is een kind van bijna twee jaar oud overleden aan de gevolgen van varkensgriep. Het is het eerste sterfgeval door deze ziekte buiten Mexico.

In New York hebben studenten die niet in Mexico zijn geweest ondertussen toch varkensgriep gekregen. Zij moeten daarom door klasgenoten die naar Mexico zijn geweest zijn aangestoken.

The New York Times meldt: “In France, the health minister took the extraordinary step of calling for a suspension of all flights from the European Union to Mexico, the epicenter of the outbreak, even as a Mexican health official said that the death toll appeared to be stabilizing.”

De krant schrijft verder: “More than 150 people are suspected to have died from the virus in Mexico, and at least 2,400 people are suspected to have been infected.”

Dementerende ouderen in Eindhoven kunnen voortaan herinneringen ophalen met het spel De Klessebessers.

Dat bericht het dagblad Trouw. Met een radio, een nostalgische televisie, een oude telefoon en een doos met beelden en geluidsfragmenten van vroeger wil men zo gesprekken op gang proberen te brengen tussen de mensen.

The deadly H1N1 influenza virus that’s fueling fears of a global pandemic is a hybrid of two common pig flu strains, scientists who have studied the disease told Tuesday. Earlier reports called it a combination of pig, human and avian influenza strains, Wired Science reports.

UN SWINE FLU PANDEMIC ALERT REMAINS AT PHASE 4 AMID RISING INFECTIONS

The international influenza pandemic alert remains at phase four, on a six-level warning scale, the United Nations health agency said today, as the count of swine flu infections continues to rise.

The World Health Organization (WHO) reported that at present there are 79 cases – up from yesterday’s tally of 73 – worldwide of the strain of the new H1N1 influenza, otherwise known as swine flu because the strain is thought to have originated from pigs.

The virus has infected 40 people across the United States, 26 in Mexico and 6 in Canada, according to WHO, which also noted that there are verified reports of 2 cases in Spain, 2 in the United Kingdom and 3 in New Zealand.

“There have been seven deaths among this total number of cases, all of which are reported in Mexico,” WHO Assistant Director-General Keiji Fukuda told reporters in Geneva.

Mr. Fukuda pointed out that the number of swine flu infections WHO reports are only of laboratory confirmed cases specific to the virus and not the suspected numbers of H1N1 human contaminations presented in the media.

Although WHO has determined that the disease has spread to a number of different countries, it is only in Mexico that swine flu has reached sustained levels of person-to-person infections, which warrants a phase 4 alert.

Mr. Fukuda highlighted that the phase 4 alert recognises the increased risk of a pandemic, but does not mean the virus will necessarily spread to different countries and be transmitted to large numbers of people.

“It is too early to say that pandemic is inevitable but it is a possibility,” he said, adding that it is also too early to determine the origins of this new strain of the influenza virus.
He noted there is still no good explanation as to why cases of the infection in Mexico have appeared more severe than in other countries.

Although the evolution of the outbreak into a pandemic is not certain, Mr. Fukuda underscored the importance for countries to prepare themselves for the possibility of the global spread of the disease.

“We are beginning to focus on the anticipated needs of developing countries if the pandemic should develop,” he said. “We know from the analysis of past pandemics and we also know from many infectious diseases and health problems that the poor and the developing countries are the ones that really get hit the hardest.”

Developing countries also have the least resources to deal with a health emergency, warned Mr. Fukuda, while calling on the international community to help meet the demands a crisis may place on the poor.

WHO is working to facilitate the process needed to develop a vaccine effective against the swine flu virus, which the agency noted could take around 4 to 6 months. In addition, several more months would be needed to produce a suitably large amount of the vaccine.

Vogelpest en Hongkong griep
Sinds begin 2004 zijn er meer dan 65 mensen overleden aan het vogelpestvirus. Varianten ervan zijn zeer gevaarlijk. Daarnaast kunnen er mensen aan zijn overleden zonder dat dit ooit is vastgesteld.

Een bestaand virus kan evolueren in een subtype. Dat laatste is gebeurd met het H2N2 virus dat uiteindelijk evolueerde in het subtype H3N2.

Hierdoor brak de Hongkong griep uit in 1968, die een wereldwijde pandemie veroorzaakte, waarbij volgens verschillende bronnen zeker 500.000 dodelijke slachtoffers vielen.

Situatie varkensgriep wereldwijd alarmerend, Tamiflu blijkt effectief

Situatie in Nederland:

De Volkskrant meldt: “De verschijnselen van de varkensgriep uit Mexico zijn dezelfde als van normale seizoensgriep: koorts, snotteren en eventueel gewrichtspijn. Het RIVM raadt mensen met griepverschijnselen die recent in Mexico zijn geweest naar hun huisarts te gaan. Die kan een sputummonster laten analyseren op de aanwezigheid van influenzavirus."

De krant vervolgt: "De uitslag daarvan is binnen twaalf uur beschikbaar, aldus Jim van Steenbergen van de Landelijke Coördinatie Infectieziektebestrijding (LCI). Is die uitslag positief, dan kan een virusremmer zoals Tamiflu worden voorgeschreven. "

Tamiflu blijkt effectief tegen de Mexicostam te werken. Dit blijkt uit Amerikaanse onderzoek. In Nederland hebben zich intussen 4 mensen gemeldt die naar Mexico zijn gereisd die griepverschijnselen hebben. Nader onderzoek heeft niets opgeleverd, aldus de LCI.

Amerikaanse media hebben intussen gemeld dat er in de VS 40 gevallen zijn. Nederlandse media blijken dus traag.

The Los Angeles Times meldt maandag dat er enkele tientallen nieuwe besmettingen met varkensgriep in de VS zijn geconstateerd. Het gaat om lichte vormen van de infectie. In totaal is er in de VS 1 persoon in het ziekenhuis opgenomen.

“20 of the infected, confirmed overnight, are in New York and one in California. The new cases are mild, officials say. The virus, which has spread to Europe, is suspected in 149 deaths in Mexico.”

“Twenty-one new cases of swine flu were confirmed overnight in the United States, 20 in New York and one in Sacramento County, health officials said today,” aldus de krant.

“The new cases were mild, and there has still been only one hospitalization in this country, that of a woman in Imperial County, a spokesman for the organization noted. She has since recovered.”

Oseltamivir is een antiviraal geneesmiddel. Het wordt door Roche op de markt gebracht onder de merknaam Tamiflu. Het is werkzaam tegen influenzavirussen. Door het gebruik van Tamiflu wordt de vermenigvuldiging van virussen in het lichaam verminderd, zodat geïnfecteerde lichaamscellen geen andere cellen besmetten.

UN CONSIDERING RAISING GLOBAL PANDEMIC ALERT LEVEL DUE TO SWINE FLU SPREAD

With swine flu infections now confirmed in four different countries, the United Nations health agency may raise the international pandemic alert today to phase 4 or 5 – out of six – for the first time since the current warning system was introduced in 2005 for the avian influenza crisis.

The emergency committee, established by the World Health Organization (WHO) to advise on the recent outbreak of the deadly new strand of the flu virus, is currently meeting to determine the status of the pandemic alert level.

WHO reported that there are confirmed cases in Canada, Mexico, Spain and the United States, and there are rumours of the infection spreading to others.

“We now have 40 reported cases in the United States and approximately 26 in Mexico, others in Canada and we now have a verified case in Spain,” a WHO spokesperson told reporters in Geneva.

Phase 5 of the WHO pandemic alert levels is characterized by human-to-human spread of the virus into at least two countries in one WHO region.

“The declaration of Phase 5 is a strong signal that a pandemic is imminent and that the time to finalize the organization, communication, and implementation of the planned mitigation measures is short,” says the agency’s website.

The geographic spread and the increasing number of human cases of swine influenza A (H1N1) has brought forward the second meeting of the WHO emergency committee to today.

Secretary-General Ban Ki-moon also expressed concern over the virus, noting that under WHO Director-General Margaret Chan, “the UN system is actively working with all concerned Governments, especially those of Mexico and the United States, to rapidly scale up support in controlling the outbreak.”
